Complete Flood Damage Questionnaire

Complete Flood Damage Questionnaire

This form is not a form for financial assistance or a FEMA application for assistance.

At this time, the County of Midland is waiting for the results of the FEMA assessment. Until the results and recommendations are in, we will not know what/if any assistance will be available to the community. This process can take up to 30 days. Midland County Emergency Management is recommending that residents affected by the flood save their receipts for items that had to be replaced or for work that had to be done and paid for to mitigate the damage and the flood waters. They recommend keeping good documentation and photos of damages caused by the flood.

If the County of Midland is informed of any programs being made available to the community, it will be provided to the local media and posted on the County website, City website, Midland 911 website and all corresponding Facebook pages. This information will include information on what is available and how to apply. Until then, we are unable to provide answers about the types of financial assistance that will be available.

If you are a City of Midland or Midland County resident or business owner and have structural damage that was caused by the recent flood waters, or that had flood waters inside their home or business, you can complete this flood damage questionnaire.  However, Midland County Emergency Management is no longer collecting the damage assessment information.  They have collected all the information they needed to provide FEMA and the State of Michigan a report.

If you are in need of clean up resources and assistance, you need to call the United Way 2-1-1 Center by dialing 2-1-1 and advise them you are calling because you were impacted by the Midland Flood.

The Midland Area Community Foundation (MACF) and United Way of Midland County have jointly established a Disaster Relief Fund of up to $75,000 that was approved by foundation trustees.  They are offering gift cards from this fund to residents that have been affected by the flood and that have completed the damage assessment form.  These gift cards can be obtained at the Midland Area Community Foundation at 76 Ashman Circle, Midland, MI 48640.  Their regular office hours are Monday through Thursday from 8am to 5pm and Friday from 8am to 3pm. Their office hours are subject to change based on community events.  Their office number is 989-839-9661.
